It looks like you are seeing an Azure-standard message “Not Effectively Entitled” or "not assigned", which means that the user is not in scope for provisioning. This happens when you have configured the provisioning scope to "Sync assigned Users and Groups". Can you check if you have added this under the "Users & Groups" sub-menu within the Provisioning screen?
You can do it by following these steps:
1. Go to Azure Portal > Enterprise Applications > Freshservice Provisioning.
2. Click on Users & Groups.
3. Check if the user you are attempting to provision is on the list.
4. If not, add the user.
5. Proceed with provisioning on demand to test if it's working well.
