Freshservice app configuration page, beneath the Secret Token section, you can see the custom field names.

You can find this from

Freshservice Admin > Apps > Installed Apps > Azure Provisioning (SCIM) > Settings. Scroll below Secret Token, and the table contains the custom fields.
